This page contains links to the range of documents and forms required by clubs for GTAS Events in 2021.

Event Policy & Procedures


The Event Policy sets out the  parameters for holding GTAS events.

The Event Procedures are relevant for Host Clubs, participating clubs and Gymsports Committees.  It sets out the sequence of tasks for event entries and exchanges of documentation between all parties.

Entry Forms

All entries by clubs for all events must be on the relevant 2018 Entry Forms and must be submitted by the deadlines identified on the GTAS 2018 Events Calendar.  Note: Please use the 2018 forms as these have been updated.

Event Entry Refund Form

This form is for clubs to claim for athletes who have had to withdraw from events due to medical reasons. Please read clause 1.11 Athlete Withdrawal section of the 2018 Event Policy before submitting the refund form to gymtas@gymnastics.org.au

Host Club Forms

Injury Report Form 

The Injury Report Form - use Host Clubs Injury Report Form

Event Medal/Ribbon Order Form 
The Event Medal/Ribbon Order Form is to be completed by the Host Club and sent to GTAS Administration 2 weeks prior to the event after receipt of final entries.

Post Event Report Form
The Post Event Report Form and associated documentation must be completed by the Host Club and sent to GTAS Administration as soon as possible after the event (not greater than 1 week later).

LEVELS BADGES
To order levels badges for WAG, MAG, TRP, RG click on the link below for the order form and then purchase the badges using one of these methods:

1. POST ORDER- Simply choose the goods you wish to order from the GymShop, print out our Order Form (please see below) and send your completed order with cheque, money order or credit card details (MasterCard or VISA ONLY) to Sports House, Level 2 / 375 Albert Road, Albert Park VIC 3206.  

2. FAX ORDER- Simply choose the goods you wish to order from the GymShop, print out our Order Form (please see below) and send your completed order with credit card details (MasterCard or VISA ) to (03) 8698 9799 . Alternatively you can e-mail your order to gymshop@gymnastics.org.au

3. PAYMENT: All orders must be paid in full prior to goods being sent out. Please provide credit card details on the order form, or send a cheque/money order in with the Order Form. Gymnastics Australia will no longer be invoicing affiliated Gymnastics Clubs. Schools can request an invoice – and if requested, a purchase order must be enclosed.

Postage and handling charges now apply. Standard domestic postage cost is $15.00. Express postage is $25, up to 1kg. Please contact the Gymshop for bulk postage rates over 1kg.

Badges are at a cost of $9.00 per badge.
