2017 Event Documents and Forms

This page contains links to the range of documents and forms required by clubs for GTAS Events in 2017.

Event Policy & Procedures


The Event Policy sets out the  parameters for holding GTAS events.

The Event Procedures are relevant for Host Clubs, participating clubs and Gymsports Committees.  It sets out the sequence of tasks for event entries and exchanges of documentation between all parties.

Entry Forms

All entries by clubs for all events must be on the relevant 2017 Entry Forms and must be submitted by the deadlines identified on the GTAS 2017 Events Calendar.  Note: The fees for 2017 have changed slightly for 2016; these are detailed within the Event Entry Forms.

Event Entry Refund Form

This form is for clubs to claim for athletes who have had to withdraw from events due to medical reasons. Please read clause 1.11 Athlete Withdrawal section of the 2016 Event Policy before submitting the refund form to gymtas@gymnastics.org.au

Host Club Forms

Injury Report Form 

The Injury Report Form (from JLT Insurance) can be found HERE

Event Medal/Ribbon Order Form 
The Event Medal/Ribbon Order Form is to be completed by the Host Club and sent to GTAS Administration 2 weeks prior to the event after receipt of final entries.

Post Event Report Form
The Post Event Report Form and associated documentation must be completed by the Host Club and sent to GTAS Administration as soon as possible after the event (not greater than 1 week later).
